This role is pivotal in providing top-tier nursing care in an outpatient setting, supporting both patients and medical staff. The ideal candidate will bring a proactive and compassionate approach to patient care, adhering to the highest standards of nursing practices tailored to outpatient care and chronic disease management. Provide direct and comprehensive nursing care to patients in an outpatient setting. Assist in the evaluation of patient needs and treatment plans. Administer medications and injections as prescribed. Record patients’ medical histories and monitor vital signs. Educate patients and their families on health maintenance and disease prevention. Collaborate with healthcare providers to optimize patient care plans. Ensure compliance with healthcare laws and standards. Strong clinical skills in an outpatient healthcare setting. Excellent patient management abilities. Proficient in using medical software and electronic health records. Strong communication and interpersonal skills. Medical Insurance Dental Insurance Vision Insurance Valid LPN license in the state of Vermont. CPR certified.
