Registered Nurse (RN) | Home Health Location: Newton Center, MA Agency: Cynet Health Pay: $2,760 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 4/5/2026 About the Position Job Title: RN Home Health Profession: Registered Nurse Specialty: Home Health Duration: 13 weeks Shift: Day Shift Hours per Shift: 8 hours Experience: 1 year License: Registered Nurse License Certifications: None specified Must-Have: 1 year Certified Home Care experience Description: ALL APPLICANTS MUST HAVE 1 YEAR CERTIFIED HOME CARE EXPERIENCE. 1 YEAR CASE MANAGEMENT EXPERIENCE IS REQUIRED. Ability to travel to cases as assigned is essential. Weekend rotations occur once every 6 weeks. As a Home Health RN, responsibilities include assessing patient conditions. Implementing care plans is also a key responsibility. Providing ongoing education and support to patients and their families is expected. Patient care is delivered within the organization's philosophy, policy, and standards of community health practice. Perform admission processes and case management responsibilities in accordance with established policies and standards of practice. Utilize thorough and timely electronic documentation and interdisciplinary communications. This role may involve various cases and patient interactions as assigned.
