What You Will Do
Manages and supports a clinical team in clinical programmatic and
service delivery components for a PCL region. Practices
organization standards of care and policies related to service
delivery. Participates in quality assurance measure and monitors
the delivery of PCL services. Supports compliance with
nursing/clinical standards, contracts, federal, state, and local
government regulations, and company policies.
- Monitors and evaluates employee performance and holds employees
accountable for the attainment of defined goals and objectives.
Executes effective performance redirection when needed and develops
employees through coaching, mentoring, and formal/on the job
training and development opportunities.
- Performs on-site Orientation, Supervision Visits, Skill Checks
and Annual Performances of home care aides in compliance with
Federal, State and applicable Accrediting body Regulations and AHC
- Supervises Home Care Aides in field by providing ongoing
instruction during field visits including reinforcement of Standard
Precautions, Client Respect, HIPAA and Safety; supports education
and training program.
- Develops and reviews Personal Care Plans from all contract/
payor sources, orienting aides in following the care plan as
authorized by the contract/ payor source nursing staff.
- Anticipates changing staffing needs and assists and supports
agency recruitment, hiring and retention of the Home Care Aide
staff. Develops and supports staffing plans during emergency needs
due to weather or other conditions.
- Participates in the agency Quality Improvement Program by
investigating, developing solutions, and overseeing nursing staff
follow-up, regarding incident and service complaints, reporting
incidents to local, state, federal and contract stakeholders as
- Follows the appropriate Care Plan in collaboration the Director
of Nursing, patient/client/caregivers, the patient/client's
physician and other care team members as appropriate and according
to the agency policy.
- Performs other duties as assigned.
- Graduate of an approved school of professional nursing
- Current Registered Nurse license in practicing state
- Two years working as a licensed RN required
- Two (2+) years of supervisory or management
- Two (2+) years of experience in Elder Long Term Care
specifically in home care, community health nursing or related
