Job Description:
Description The Department of Mental Health (DMH) is seeking
qualified professionals for the position of Child/Adolescent Human
Services Coordinator II Supervisor to promote services that are
clinically appropriate, least restrictive and delivered close to
home. The services are designed to be strength-based,
recovery-oriented and culturally/linguistically competent.
The selected candidate assists DMH leadership team in developing
plans for the service system, serves as a DMH system integration
specialist and liaison to Childrens Behavioral Health Initiative
(CBHI), Community Service Agencies (CSAs) and community providers
as well as coordinates referrals from Service Authorization to
providers and case managers, and provides supervision to case
management as needed.
Please Note: This is a full-time, 37.5 hours per week position, the
hours are 9:00am-5:00pm with Saturday and Sunday as days off.
Duties and Responsibilities (these are a general summary and not
all inclusive):
* Provides education to CSA staff about DMH services, including
clinical criteria and specific service authorization criteria.
* Authorizes access to DMH community resources as available for
youth receiving Intensive Care Coordination ICC services through
CBHI.
* Facilitates DMH service application process for youth in need of
services beyond those provided by CBHI.
* Provides consultation to CSA care planning teams, including
attending care planning team as needed.
* Actively participates in local system of care meetings.
* Maintains requested data related to CSA/DMH interface.
* Provides weekly individual or group supervision to Human Service
Coordinator I (HSC I) to assist, monitor and provide feedback in
order to ensure that quality services are provided to
consumers.
* Trains and orients new staff to DMH case management policies and
procedures and to services in the local Site and North East
Area.
* Educates all interested parties of the referral and eligibility
determination process and completes initial screening of
referrals.
* Provides consultation to families, other agencies and service
providers regarding DMH and community mental health services.
* Monitors client wait list and prioritizes case assignment and
approves case closings to HSC I.
* Monitors consumer records to assure that all necessary
documentation is complete and up-to-date.
* Develops and maintains working relationships with community and
other state agencies, in order to identify unmet needs and to
effectively access a comprehensive range of services for
consumers.
* Provides case management services to consumers including
providing transportation to consumers when necessary.
* Assumes and/or delegates responsibility for supervisees' clients
when supervisee is absent or unavailable, assuring continuity of
services to consumers.
* Participates in the development of clinical and administrative
policy through membership in the North East Area Child/ Adolescent
Committee and involvement in special projects as needed.
* Participates in bi-weekly supervision with the Site Director or
designee.
* Prepare/maintain statistical, administrative and narrative
reports as needed to document services and assist in program
planning.
Required Qualifications:
* Demonstrated knowledge of psychosocial problems of those with
mental/emotional disorders and the types and symptoms.
* Knowledge of the principles, practices and techniques of
counseling.
* Familiarity with the types of programs and services available to
individuals living with mental illness.
* General understanding of interviewing techniques.
* Knowledge of methods of general report writing with the ability
to assess case management process and results.
Preferred Qualifications:
* Strong and effective interpersonal skills, including the ability
to listen well, relate to others and demonstrate sensitivity and
respect for individuals.
* Demonstrated commitment to serving a diverse population with
cultural competence and sensitivity, as well as the ability to work
with a wide range of constituencies with diplomacy and tact.
* Proficiency in Microsoft Office applications including Outlook,
Word, and Excel.
* Master's Degree in social work, counseling, related field
preferred
* Given the population served, bilingual or multilingual
fluency.
Working Conditions:
* This is a full-time, 37.5 hours per week position, the hours are
9:00am-5:00pm with Saturday and Sunday as days off.
* This position travels to meet with clients and family members
outside the office in various settings such as homes, hospitals,
programs and other accessible community settings. Candidates will
need access to a reliable motor vehicle for transportation.

Qualifications First consideration will be given to those
applicants that apply within the first 14 days. Based on assignment
as a Mental Health Case Manager or Forensic Transition Team
Coordinator, within the Department of Mental Health, a Bachelor's
degree or higher in social work, psychology, sociology, counseling,
counseling education, education of the physically or emotionally
handicapped, education of the multiple handicapped, education of
the learning disabled, human services, rehabilitation counseling,
education or other a related field is required. In addition
applicants must have at least four (4) years of full-time, or
equivalent part-time, case management of clinical/therapeutic
experience. Based on assignment, travel may be required. Incumbents
who elect to use a motor vehicle for travel must have a current and
valid motor vehicle driver's license at a class level specific to
assignment.
