consumers place in us. GENERAL DESCRIPTION: This vital role offers
a unique opportunity to directly support the community resources
program. You’ll be part of a team, accompanying the Liaison on
facility visits, and handling complex cases that make a real
difference. You’ll uphold the highest standards set by the
Executive Office of Aging and Independence (AGE), ensuring all case
documentation is comprehensive and accurate. Requirements: ROLE AND
RESPONSIBILITIES: Supports the Community Transition Liaison to
operate at the top of their role and responsibility. Provides
additional support to the Liaison for complex cases. Conduct 1:1
visit with Consumers/HCP’s to gather documentation needed to assist
the consumer in applying for public benefits. Assists the consumer
in completing and submitting housing, MassHealth, and other
necessary applications. Processes appropriate referrals for CTLP,
MFP Demo, and MFP Waivers. Complete on-site skilled nursing
facility visits and accompanies Liaisons per AGE guidelines.
Conducts clerical duties as needed by Liaison and/or Supervisor,
including data entry into AGE designated cloud-based data
enterprise system. Complete weekly/monthly reports to support the
CTLP Manager. Ensures case documentation meets the standards set
forth by the Executive Office of Aging and Independence for the
Community Transition Liaison Program Representing ASNCM at Health
and/or Resource Fairs; and Perform all other duties as assigned
QUALIFICATIONS AND EDUCATION REQUIREMENTS: High School diploma or
GED with 1 year of related work experience and/or interest in
working with nursing facility residents including older adults or
persons with disabilities who are transitioning from an
institutional to a community setting. The ideal candidate will have
the ability: To independently organize work and meet deadlines. To
multi-task in a busy environment To manage constant change with a
positive attitude To articulate program needs and goals and make
public presentations. To positively influence others to achieve
results that are in the best interest of the organization. Must be
proficient in Microsoft Office A valid driver's license and a
legally insured and registered motor vehicle are required, a copy
of current license and registration will be on file in the
personnel record. ESSENTIAL FUNCTIONS: Sufficient clarity of speech
and hearing which permits communication with supervisor,
co-workers, consumers, community professionals, and general public.
Ability to talk on a phone, stand, walk, sit, and travel in the
community. Ability to travel throughout the state of Massachusetts
in a private vehicle under all weather conditions including snow.
Ability to express ideas concisely and clearly, orally and in
writing. Sufficient vision which permits production and review of a
wide variety of materials, written correspondence, reports and
related electronic and paper communication. Ability to lift and
carry objects weighing 20 pounds. Ability to read and analyze data.
Ability to exercise sound judgment in making decisions. The above
